Usage
Place this template on the user page (not the user talk page) of the IP editor.

Other notes
Indefinitely blocked IP addresses should be tagged with {{Blocked user|ip}} so that they are included in Category:Indefinitely blocked IP addresses.
This template automatically adds to the correct category specified by the user.
It also adds the __NOINDEX__ magic word.
